Forms for requesting an absence from school for your child should be completed by the Parent/Guardian and forwarded to the school not less than one month prior to the period of absence requested, and returned to the school office. 

• Administering medicines form Please complete this form online if you wish for a memebr of staff to administer medicine

• Leave of Absence Request form Please complete this form online if you wish to apply for leave of absence during term time.
